Does Vermont Medicaid Cover Wegovy or Zepbound?

By Anthony K C Fong·Last reviewed:

Vermont (VT) GLP-1 access depends on three state-specific policy questions before it ever gets to pricing: does Vermont Medicaid cover weight-loss GLP-1s, has Vermont expanded Medicaid under the ACA, and does the state employee health plan cover them. Answering those first tells you whether you're likely working with insurance or planning for cash-pay.

Vermont Medicaid GLP-1 coverage: what the state's own documents say

Vermont's Medicaid preferred drug list states that drugs used for weight loss are excluded from coverage; Wegovy is covered only for cardiovascular risk reduction (BMI over 27 plus a prior heart attack, stroke or symptomatic peripheral artery disease) or MASH, and Zepbound only for moderate-to-severe obstructive sleep apnea.

Vermont state employee health plan: what the plan's own documents say

Not verified. No State of Vermont document that could be opened says whether weight-loss GLP-1s are covered. The medical plan excludes 'Treatment of obesity, except surgical treatment' but pharmacy is a separate plan whose document adopts the pharmacy benefit manager's formulary, and the Express Scripts 2026 formulary the State posts lists Wegovy and Zepbound pens as formulary drugs (with Saxenda and Zepbound vials excluded) while warning that not all listed drugs are covered by all plans. Nothing State-specific shows whether weight-loss coverage was elected.

Why GLP-1 coverage varies this much by state

Medicaid is a state-federal partnership, not a single national program. Each state Medicaid agency sets its own preferred drug list (PDL) and prior-authorization criteria within federal minimum requirements. Weight-loss drugs are not a federally mandated Medicaid benefit, so states choose independently whether to cover them - which is why only 10 of 50 states currently do (Vermont is not one of them).

State employee health plans are a useful bellwether. These plans cover hundreds of thousands of state workers and negotiate directly with pharmacy benefit managers, so their coverage decisions often signal where commercial insurers in that state are heading. North Carolina's State Health Plan, for example, ended coverage of weight-loss GLP-1s on April 1, 2024, after its board voted 4-3 to cut it citing the drugs' high cost - a case that other states and large employers have watched closely.

Medicaid expansion status shapes who even has coverage to begin with. States that haven't adopted ACA Medicaid expansion have narrower Medicaid eligibility overall, which - combined with the weight-loss GLP-1 exclusion most states apply regardless of expansion status - pushes a larger share of residents in those states toward commercial insurance, telehealth, or manufacturer self-pay as their only realistic path.

National reference pricing (same nationwide - your actual cost depends on your plan)

These prices are the same across all 50 states because manufacturer list prices, self-pay programs, and telehealth pricing don't vary by state - only insurance coverage and copay do, which is why the Vermont-specific policy facts above matter more than the table below for predicting your actual cost.

Frequently asked questions

Does Vermont Medicaid cover Wegovy or Zepbound for weight loss?

No. This was checked against Vermont's own documents on September 20, 2026; coverage rules change often, so confirm with your plan before relying on it. Coverage for type 2 diabetes (Ozempic, Mounjaro) is a separate question. Medicare's GLP-1 Bridge, which started July 1, 2026, works separately from Medicaid and applies in every state.

Has Vermont expanded Medicaid under the ACA?

Yes - Vermont has adopted Medicaid expansion, which broadens who qualifies for Medicaid coverage generally, though it does not by itself guarantee GLP-1 weight-loss coverage (see above).

Does the Vermont state employee health plan cover Wegovy or Zepbound?

We could not verify this. Check the current plan documents or ask the plan directly.
