Does North Carolina Medicaid Cover Wegovy or Zepbound?

By Anthony K C Fong·Last reviewed:

North Carolina (NC) GLP-1 access depends on three state-specific policy questions before it ever gets to pricing: does North Carolina Medicaid cover weight-loss GLP-1s, has North Carolina expanded Medicaid under the ACA, and does the state employee health plan cover them. Answering those first tells you whether you're likely working with insurance or planning for cash-pay.

North Carolina Medicaid GLP-1 coverage: what the state's own documents say

NC Medicaid covers GLP-1s for obesity again since December 12, 2025, with prior authorization: Wegovy (pen and tablet) is preferred; Zepbound, Saxenda and generic liraglutide are non-preferred. It had discontinued coverage October 1 to December 11, 2025. A 2026 state law directs a plan for new utilization controls no earlier than July 1, 2027.

North Carolina state employee health plan: what the plan's own documents say

North Carolina's State Health Plan has excluded weight-loss use of GLP-1 and GIP/GLP-1 drugs (Wegovy, Saxenda, Zepbound) for all members since April 1, 2024, after its Board of Trustees voted 4-3 on January 25, 2024. Diabetes GLP-1s remain covered. The plan's July 2026 formulary lists no Wegovy, Zepbound, Saxenda or Foundayo, and its preferred-drug list names orlistat and Qsymia (with prior authorization) as alternatives to Zepbound.

Why GLP-1 coverage varies this much by state

Medicaid is a state-federal partnership, not a single national program. Each state Medicaid agency sets its own preferred drug list (PDL) and prior-authorization criteria within federal minimum requirements. Weight-loss drugs are not a federally mandated Medicaid benefit, so states choose independently whether to cover them - which is why only 10 of 50 states currently do (North Carolina is one of them).

State employee health plans are a useful bellwether. These plans cover hundreds of thousands of state workers and negotiate directly with pharmacy benefit managers, so their coverage decisions often signal where commercial insurers in that state are heading. North Carolina's State Health Plan, for example, ended coverage of weight-loss GLP-1s on April 1, 2024, after its board voted 4-3 to cut it citing the drugs' high cost - a case that other states and large employers have watched closely.

Medicaid expansion status shapes who even has coverage to begin with. States that haven't adopted ACA Medicaid expansion have narrower Medicaid eligibility overall, which - combined with the weight-loss GLP-1 exclusion most states apply regardless of expansion status - pushes a larger share of residents in those states toward commercial insurance, telehealth, or manufacturer self-pay as their only realistic path.

National reference pricing (same nationwide - your actual cost depends on your plan)

These prices are the same across all 50 states because manufacturer list prices, self-pay programs, and telehealth pricing don't vary by state - only insurance coverage and copay do, which is why the North Carolina-specific policy facts above matter more than the table below for predicting your actual cost.

Frequently asked questions

Does North Carolina Medicaid cover Wegovy or Zepbound for weight loss?

Yes. Coverage for type 2 diabetes (Ozempic, Mounjaro) is a separate question. Medicare's GLP-1 Bridge, which started July 1, 2026, works separately from Medicaid and applies in every state.

Has North Carolina expanded Medicaid under the ACA?

Yes - North Carolina has adopted Medicaid expansion, which broadens who qualifies for Medicaid coverage generally, though it does not by itself guarantee GLP-1 weight-loss coverage (see above).

Does the North Carolina state employee health plan cover Wegovy or Zepbound?
