Does Minnesota Medicaid Cover Wegovy or Zepbound?

By Anthony K C Fong·Last reviewed:

Minnesota (MN) GLP-1 access depends on three state-specific policy questions before it ever gets to pricing: does Minnesota Medicaid cover weight-loss GLP-1s, has Minnesota expanded Medicaid under the ACA, and does the state employee health plan cover them. Answering those first tells you whether you're likely working with insurance or planning for cash-pay.

Minnesota Medicaid GLP-1 coverage: what the state's own documents say

Minnesota Medical Assistance lists Saxenda and Wegovy as preferred and Zepbound as non-preferred weight management agents on the DHS Uniform Preferred Drug List effective May 7, 2026, with prior authorization; Minnesota's own criteria page could not be read, so the eligibility criteria are not independently confirmed.

Minnesota state employee health plan: what the plan's own documents say

Not verified for 2026. The SEGIP 2026 Summary of Benefits never names GLP-1s (drugs run through the CVS Caremark formulary with prior authorization, and weight-loss drugs are excluded 'unless Medically Necessary, appropriate Treatment, and a Plan-approved program'); older secondary sources show weight-loss GLP-1s covered through prior authorization into 2025, but no 2026 primary criteria document could be retrieved.

Notable insurers active in Minnesota (not independently verified): Blue Cross Blue Shield of Minnesota, HealthPartners, UCare.

Why GLP-1 coverage varies this much by state

Medicaid is a state-federal partnership, not a single national program. Each state Medicaid agency sets its own preferred drug list (PDL) and prior-authorization criteria within federal minimum requirements. Weight-loss drugs are not a federally mandated Medicaid benefit, so states choose independently whether to cover them - which is why only 10 of 50 states currently do (Minnesota is one of them).

State employee health plans are a useful bellwether. These plans cover hundreds of thousands of state workers and negotiate directly with pharmacy benefit managers, so their coverage decisions often signal where commercial insurers in that state are heading. North Carolina's State Health Plan, for example, ended coverage of weight-loss GLP-1s on April 1, 2024, after its board voted 4-3 to cut it citing the drugs' high cost - a case that other states and large employers have watched closely.

Medicaid expansion status shapes who even has coverage to begin with. States that haven't adopted ACA Medicaid expansion have narrower Medicaid eligibility overall, which - combined with the weight-loss GLP-1 exclusion most states apply regardless of expansion status - pushes a larger share of residents in those states toward commercial insurance, telehealth, or manufacturer self-pay as their only realistic path.

National reference pricing (same nationwide - your actual cost depends on your plan)

These prices are the same across all 50 states because manufacturer list prices, self-pay programs, and telehealth pricing don't vary by state - only insurance coverage and copay do, which is why the Minnesota-specific policy facts above matter more than the table below for predicting your actual cost.

Frequently asked questions

Does Minnesota Medicaid cover Wegovy or Zepbound for weight loss?

Yes. This was checked against Minnesota's own documents on September 20, 2026; coverage rules change often, so confirm with your plan before relying on it. Coverage for type 2 diabetes (Ozempic, Mounjaro) is a separate question. Medicare's GLP-1 Bridge, which started July 1, 2026, works separately from Medicaid and applies in every state.

Has Minnesota expanded Medicaid under the ACA?

Yes - Minnesota has adopted Medicaid expansion, which broadens who qualifies for Medicaid coverage generally, though it does not by itself guarantee GLP-1 weight-loss coverage (see above).

Does the Minnesota state employee health plan cover Wegovy or Zepbound?

We could not verify this. Check the current plan documents or ask the plan directly.
