Trade-off: convenience vs commitment. The pill is daily (must be taken on an empty stomach with a small sip of water, then 30-minute fast before food); the injection is weekly. Many patients find the once-weekly injection easier to stick with long-term because they only have to think about it 4 times per month instead of 30. But for needle-averse patients or those who travel frequently with TSA-related injection-storage hassle, the pill is a clear win. Effectiveness gap is small (1-2 percentage points). Food, beverages, and other meds in the GI tract dramatically reduce semaglutide absorption. For most people this means setting an alarm 30 minutes before breakfast time and taking the pill then. Skipping doses or taking with food significantly weakens the drug.

Does Medicare cover oral Wegovy?

Same rule as injection Wegovy: Medicare Part D does not cover GLP-1s prescribed solely for weight loss (Medicare Modernization Act of 2003 statutory exclusion). The 2024 cardiovascular indication for Wegovy injection (for ASCVD patients) extends to oral Wegovy too in principle, though as of May 2026 most Part D plans have not yet updated formularies.
